Here's what we'll need from you to get started:

  1. Signed Agency Agreement Download the Agreement
  2. A copy of the declarations page of your agency's current E&O policy or a certificate of insurance for this coverage
  3. A copy of your agency's resident license
  4. A completed W-9 form for your agency Download the Form
  5. Fax or Email the above information to your local RPS Branch.

Once you have completed the steps above, you will be able to apply for available coverages by using our forms and applications.
